SECTION 1:     General Provisions (APPLICANT to read and sign)
1 Permit will not be granted unless all planned construction/development, including any work
already begun, is compliant with the applicable requirements stated herein.
2 Work at the site prior to issuance of conditional permit is solely at the risk of the applicant.
3 The permit may be revoked if any false statements are made herein.
4 The permit will expire if no work is commenced within six months of issuance.
5 Applicant is hereby informed that other permits may be required to fulfill local, state, and
regulatory requirements.
6 Frequently, base flood elevation data must be determined for construction and/or development
in a special flood hazard area.   Applicant agrees to obtain the services of a registered engineer
or land surveyor for purposes of calculating and certifying any required base flood elevations.
7 Applicant hereby gives consent to the White County Floodplain Administrator or his/her
representative to make reasonable inspections required to verify compliance.
